Trespass Enforcement Request Forms
Click here to open the form that you need to fill out. These forms must be notarized in order for the LMPD to enforce the trespass laws and for the court to prosecute.
Once you have received this form back after your initial input, please print it, sign it and notarize the form. Once you have this completed, click here to submit the signed and notarized form.
Once we have received your signed and notarized form, we will review for correctness. Once we have made sure that all information has been completed and the form is signed and notarized, you will receive a notice back that your waiver has been approved. These approved waivers are good for 2 years from the date of approval.
